Registration Process
1. The following completed enrollment forms are to be returned to the church campus office for grades K2-8 and to the west campus office for grades 9-12.

2. Applicants must turn in the following forms to the school office to be a part of their student file at the time they bring the Enrollment Application forms:
- Current original immunization form (expiration date must be current through end of school year – June, 2012)
- Copy of birth certificate
- Copy of social security card
- Copy of health insurance card
- Copy of custody papers, if applicable
- Academic records: report cards for current year and previous two years; achievement test scores for past two years
3. Every student, K-2 - 12th grade, pays a $450.00 non-refundable admission fee. A deposit of $150.00 is due when application is submitted (new students will need to include a $25.00 testing fee) with the remaining $300.00 balance to be paid by March 15, 2012. After March 15, 2012 the $300.00 balance will be added to your tuition.
Entrance test date/interview time will be given upon receipt of all paperwork.
