Registration Process
1. The following completed enrollment forms are to be returned to the church campus office for grades K2-8 and to the west campus office for grades 9-12.
- New Student Enrollment Application Form
- Parental Financial Contract
- Parental Commitment Form
- Family Information Forms
- Principal/Counselor Recommendation Form
Print all forms listed above
2. Applicants must turn in the following forms to the school office to be a part of their student file at the time they bring the Enrollment Application forms:
- Current original immunization form (expiration date must be current through end of school year – June, 2011)
- Copy of birth certificate
- Copy of social security card
- Copy of health insurance card
- Copy of custody papers, if applicable
- Academic records: report cards for current year and previous two years; achievement test scores for past two years
3. Every student pays a $450.00 enrollment fee. The $150.00 application fee portion is due when applying, along with a $25.00 entrance test fee. The $300.00 balance is due upon receipt of a letter of acceptance from CHCA. Students registering after March 15 must pay the full $450.00 enrollment fee and $25.00 entrance test fee at the time of application.
Entrance test date/interview time will be given upon receipt of all paperwork.

Registration for the public opens and will close when all classes are filled |
